Winston's daily struggles through his life in 1984 are full of stress and fear, with little control of his own actions. His life is powerless and futile; he does everything he is told without questions ; a lot like peer pressure to kids now in present time. Winston chooses to do cigarettes to escape his deselect world and even if just for a short time, live in a state of bliss.
Just like in the real world outside of 1984 people are under the control, maybe not by the government but under the control of nicotine and its effects. In 1984, Winston took cigarettes to calm him down thanks to "tobacco to help manage unpleasant feelings and emotions.."
He was able to somewhat escape the cruel world and its harshness. Despite how horrible they may have been he used them as a part of a stress reliever and felt the need to do so since rations we limited and you take what you get and he got rationed cigarettes. After years of using cigarettes he became addicted to the nicotine where he as a "...smoker not only become physically dependent on nicotine. There’s also a strong emotional (psychological) dependence." Making it hard for him to escape its addiction.
